In my last game (Part 1 with no extensions activated), I'm sure I met the conditions for the following achievements, but they didn't unlock:

Interstellar Traveler - left Eridani
You Only Live Twice - claimed insurance once
Benedict and Fiona - completed all benedict quests

I have managed to unlock the last one, so its not completely broken.

Fixed Interstellar Traveler (already deployed) and You Only Live Twice (needs a client update).

Benedict and Fiona seems to be working fine.
