When visiting a remora ripper to dismantle items, the letter D is highlighted to Dismantle, but also for Done.
So when you're done dismantling broken items and press D for done, actually you will be dismantling the currently selected item. You will have to click on Done with teh mouse, or press Esc to leave the Dismantle screen.
